The BAER Project’s HEALTH CARE ASSISTANCE PROGRAM offers an online marketing solution, which includes an incentives-based marketing survey for product testing research, to gather statistical or demographic data or for product development. Each survey provides an option to place streaming video or “a commercial” for product marketing.

This information request form will allow us to evaluate your current marketing initiatives and promote your survey to our subscribers for your online marketing and information collection needs.

In order to expedite your request for information, we require that you complete all of the fields below. All information is considered confidential and will be used for internal review only. After you have submitted the form, we will carefully review the information and contact you as soon as possible.

Our monthly Email Newsletter (The BAER Report) is sent to all subscribers. Each subscriber is required to receive, review and respond to each E-Newsletter as part of the monthly guidelines for membership in the HEALTH CARE ASSISTANCE PROGRAM. Information in the E-Newsletter will be health related and all advertisers are provided with opportunities to include product or service information along with links to the advertisers website, video links or etc.
